ARRANGEMENTS FOR MEDICAL TREATMENT VISA APPLICATIONS 2015 (Item 1214A) I, MICHAELIA CASH, Assistant Minister for Immigration and Border Protection, acting under subregulation 2.07(5) of the Migration Regulations 1994 (the Regulations) for the purposes of item 1214A of Schedule 1 to the Regulations: • REVOKE Instrument number IMMI 13/116 (F2013L01668), signed on 3 September 2013, specifying the post office box address, the courier address and the fax numbers for the purposes of subparagraphs 1214A(3)(a)(i), 1214A(3)(a)(ii) and 1214A(3)(a)(iii) of Schedule 1 to the Regulations; AND Item 1214A Medical Treatment (Visitor) Class UB Subclass 602 (Medical Treatment) visa 2.    SPECIFY for the purposes of subitem 1214A(1) the following approved form and for paragraph 1214A(3)(a) the following place and manner for making an application for a Medical Treatment (Visitor) (Class UB),  Subclass 602: Form Place and Manner 48ME (1)   Application must be made by (a)    posting (with the correct prepaid postage) to: Department of Immigration and Border Protection GPO Box 717 Canberra ACT 2601 AUSTRALIA; or (b)   delivering by courier service to: Department of Immigration and Border Protection Ground Floor 3 Lonsdale Street  Braddon ACT 2612 AUSTRALIA; or (c)    faxing to the following fax numbers: 0061 (02) 6195 6077  or  0061 (02) 6195 6104 (2)   Application by an applicant who is outside Australia must be made at a diplomatic, consular or migration office maintained by or on behalf of the Commonwealth of Australia This Instrument, IMMI 15/037, commences on 18 April 2015.
