ARRANGEMENTS FOR MEDICAL TREATMENT VISA APPLICATIONS 2015

I, MICHAELIA CASH, Assistant Minister for Immigration and Border Protection, acting under subregulation 2.07(5) of the Migration Regulations 1994 (the Regulations) for the purposes of item 1214A of Schedule 1 to the Regulations:

  1. REVOKE Instrument number IMMI 13/116 (F2013L01668), signed on 3 September 2013, specifying the post office box address, the courier address and the fax numbers for the purposes of subparagraphs 1214A(3)(a)(i), 1214A(3)(a)(ii) and 1214A(3)(a)(iii) of Schedule 1 to the Regulations; AND

Item 1214A Medical Treatment (Visitor) Class UB Subclass 602 (Medical Treatment) visa

2.    SPECIFY for the purposes of subitem 1214A(1) the following approved form and for paragraph 1214A(3)(a) the following place and manner for making an application for a Medical Treatment (Visitor) (Class UB),  Subclass 602:
